(Visit get.adobe.com/reader/otherversions to download Acrobat for FREE.)
2. Download the PDFs (above) and open them in Adobe Acrobat
3. Type the information into boxes
4. Save the completed form
5. Send completed form directly to ctgllc_office@medicineathome.net
